I think some designs are more obvious than others.

there's different designs but - I think initially it is just a standard necklace, or ankle bracelet with a key on it.

The key, of course, belonging to a lock chastity device - this being of course then extra symbolic that they key is hold close to the breasts (something he is denied) or feet (either as a sign of lowliness, or to play into foot fetish)

Of course, now this has become more of a hot fashion - so you can literally buy necklaces or anklets with a key on them (which goes nowhere, presumably) but there can be other variations - like, either, literally read 'hot wife' or 'HW' (which to the unsuspecting could be someone's initials)

Really, the reality of most blokes hardly puts them into the catoagory of "hot", not even mildly warm in most cases.

there's a lot of stuff from porn has overspilled into kink or other sex - and, I think hotwife is very likely to be one of them (some googles suggest this was first used on swinging discussion boards in the 90s, of men wanting to show off their hot wife) although swinging in different shades really took off in the 60s (they don't call them the swinging 60s for nothing)
