I suspect for M/F and possibly F/F anal, its the taboo thing. It's also a dominance thing. The person being penetrated is the submissive one. This goes back to ancient Greece (powerful Greek and Roman politicians suffered not because they had sex with a man but because they were the receiver not the giver).

For M/M or preop MtF its the only way.
