In my heart its some extravagant protagonist & arch nemesis situation, like the pivotal point in their arc where they become enemies but realistically I think this refers to people into the idea of cheating or being cheated on, often to do with being caught or catching a partner in the act. I feel like I can get behind it as a fantasy or roleplay scenario but as others have pointed out it also seems to be used in contexts where there is genuine infidelity.
